Compassionate Care Connections Home Services

Welcome to Compassionate Care Connections Home Services (“Company,” “we,” “us,” or “our”). By using our services, you agree to comply with the following Terms and Conditions. Please read them carefully before engaging our services.

1. Scope of Services

Compassionate Care Connections Home Services provides non-medical home care services, including but not limited to:

  • Personal care assistance

  • Companionship services

  • Light housekeeping

  • Meal preparation

  • Medication reminders

  • Transportation assistance

2. Client Responsibilities

Clients and their families agree to:

  • Provide accurate health and personal information necessary for care.

  • Maintain a safe environment for caregivers.

  • Treat staff with respect and dignity.

  • Notify the Company of any changes in service needs or cancellations with at least [24/48] hours' notice.

3. Payment and Billing

  • Payment is due in advance or upon receipt of invoice, as per the agreed-upon schedule.

  • Late payments may incur additional fees.

  • Any additional services requested outside the standard care plan may result in extra charges.

  • The Company reserves the right to suspend or terminate services due to non-payment.

4. Cancellation and Refund Policy

  • Cancellations must be made with at least [24/48] hours’ notice to avoid service charges.

  • Refunds, if applicable, will be processed on a case-by-case basis.

  • Emergency cancellations due to hospitalization or unforeseen circumstances will be reviewed for possible fee waivers.

5. Liability and Limitation of Responsibility

  • The Company and its caregivers provide non-medical assistance only.

  • We are not liable for medical emergencies, accidents, or injuries occurring outside of our direct service scope.

  • Clients must carry their own health insurance for medical needs.

6. Confidentiality and Privacy

  • We comply with all HIPAA regulations to protect client privacy.

  • Personal information will not be shared with third parties without consent, except as required by law.

7. Caregiver Conduct

  • Caregivers are not permitted to accept gifts, loans, or money from clients outside of service payments.

  • Any concerns regarding caregiver performance should be reported to management immediately.

8. Termination of Services

The Company reserves the right to terminate services if:

  • The client engages in abusive, unsafe, or inappropriate behavior.

  • Non-payment issues arise and remain unresolved.

  • The client’s needs exceed our scope of services.

9. Governing Law

These Terms and Conditions shall be governed by the laws of the State of [Indiana], and any disputes shall be resolved in the courts of [County, State].

10. Amendments

The Company reserves the right to update these Terms and Conditions at any time. Clients will be notified of significant changes.
